A Quality Improvement Project

Our goal is to improve newborn care by increasing the number of newborn care visits where:

  1. the care provider has all the weights and bilirubin values
  2. the parents have all the weights (in grams)

We are tracking visits at Vernon Public Health and at the Centreville Clinic.

How can I help?

You can help as you work in your role. We would like to reliably do the following starting Feb 9, 2022.

Parents
  1. Please take a photo of the weight in grams each time it is taken.
  2. Bring your phone to each appointment.

Nurses on Mat-Child at VJH
  1. Please complete every newborn liaison record as you usually would.
  2. Please include the birth weight and discharge weight (where applicable) on the liaison record.
  3. Please include any TcB or TsB (where applicable) in hospital on the liaison record.
  4. Please invite parents to take a photo of the weight in grams each time it is taken.

Unit Clerks on Mat-Child at VJH
  1. Please fax every newborn record to the MRP after every discharge.

Public Health Nurses
  1. Please record visit information, including if you had all the information you needed, and if the parents had the weights.
  2. Please invite parents to take a photo of the weight in grams each time it is taken.

MOAs at the Centreville Clinic
  1. Please record visit information, including if you had all the information you needed, and if the parents had the weights.
  2. Please invite parents to take a photo of the weight in grams each time it is taken.

Physicians in the Centreville Call Group
  1. Please complete every newborn record as you usually would.
  2. Please include the birth and discharge weight (where applicable) on the newborn record.
  3. Please include any TcB or TsB (where applicable) in hospital on the newborn record.
  4. Please include a request to fax the Newborn Record to the office of the MRP at each discharge.
  5. Please invite parents to take a photo of the weight in grams each time it is taken.
